This form is used to request general information about the adoption, including the names of the birth parents, the date and place of the birth, and the date and place of the adoption. Additionally, the form can be used to request medical, social, and family history of both the adoptive and birth parents. The two types of Washington Request for Non-Identifying Adoption Information are the Adult Adopted Request for Non-Identifying Adoption Information, and the Birth Parent Request for Non-Identifying Adoption Information. The Adult Adopted Request is used by an adult adopted to request non-identifying adoption information. The Birth Parent Request is used by a birth parent to request non-identifying adoption information.
