JACC is a State-funded program that provides a broad array of in-home and community-based services to individuals age 60 and older who meet clinical eligibility for nursing home level of care and who desire to remain in their homes within the community.
Meets financial eligibility requirements: Having a monthly income that is not greater than 365% of the Federal poverty level ($4,760 for an individual; $6,433 for a married couple in 2025). Having countable assets totalling $40,000 or less for an individual or $60,000 or less for a couple.
The Division of Aging Services uses NJSave, which is one application seniors and individuals with disabilities can use to get help paying Medicare premiums, prescription costs, and other living expenses. People can apply online or via a paper application. Both are available in English and Spanish.

The New Jersey Personal Preference Program (PPP) offers a way for NJ FamilyCare/Medicaid members, who qualify for the Personal Care Assistant (PCA) service, to remain in their home, active in their community, and does not require the use of a home health care agency.
What to Include in a Checklist for Caregivers Name of caregiver on duty. Date the checklist is being used. Name of patients. Patient's morning, afternoon, and evening routines, such as: Taking medication and vital signs. Eating food and drinking water. Housekeeping tasks to do. Changes in patient's condition, if any.
