Claim forms are available by logging into the member website at blueshieldca or by contacting the benefit administrator. Please submit your claim form and medical records within one year of the service date.

Timely filing is when an insurance company put a time limit on claim submission. For example, if a insurance company has a 90-day timely filing limit that means you need to submit a claim within 90 days of the date of service.
Anthem follows the standard of: • 90 days for participating providers and facilities. 15 months for nonparticipating providers and facilities.
Contact the retailer: If you are sure the card is not activated, contact the retailer where you purchased the card. Bring the card and the receipt to the store and explain the situation. They should be able to assist you in activating the card or issuing a refund.
If you notice unauthorized charges, you will need to dispute them with the merchant directly.
After you submit the dispute, Walmart will communicate the dispute decision via email or you can track the approval status via the Disputes dashboard in Seller Center. Disputes are typically resolved within 24-48 hours of receipt. However, if additional information or approval is needed, it could take longer.
Gift Cards do not expire. 4. Returns and Refunds. Gift Cards are not returnable or refundable.
If you see unauthorized transactions, please promptly contact the financial institution that issued your card. Typically, there is a customer service number for your financial institution listed on the back of the card or on your billing statement.
