The DS-82 is printed on A4 format paper, each page on a separate sheet. Forms printed on both sizes of paper will not be accepted. The form can be printed in black and white or in color.

As we mentioned above, the Form DS-82 comes in the form of a six-page PDF file, of which the first four pages are for instructions to fill the form while the remaining two pages are the form itself.
