3 Answers 3 There is an absolute requirement that either both parents are present when applying for the US passport or a Consent is given by one of the parent's: Minors under age 16 cannot apply for a passport by themselves.
The Statement of Exigent/Special Family Circumstances for Issuance of a U.S. Passport to a Minor under Age 16 (DS-5525) is used in conjunction with the Application for a U.S. Passport (DS-11).
