DescriptionThis Revised Uniform Anatomical Gift Act Donation form pursuant to state statutes designates the specific body parts and organs an individual wishes to donate at the time of death. Any individual eighteen years of age or older and of sound mind may make an anatomical gift for any statutorily specified purpose by execution of a document of gift or by will. If a person less than eighteen years of age wishes to make an anatomical gift, one of the witnesses who signs the document shall be a parent or guardian of that person. This form may be witnessed and the signature notarized. This form also includes the necessary enrollment form for the Donor Registry managed by the Ohio Department of Motor Vehicles. | |||||||||||||||||||
