You can provide this information by labelling the allergens contained in individual dishes, or by displaying a sign directing customers to ask staff for allergen information. This information must be visible, clearly legible and easily accessible to the customer.

The Act requires that food labels must clearly identify the food source names of any ingredients that are one of the major food allergens or contain any protein derived from a major food allergen.

Place an asterisk by the name of the food and use a footnote that states the food is served raw or undercooked, or contains, or may contain, raw or undercooked ingredients. A reminder is a written statement that eating raw or undercooked food is associated with increased risk of foodborne illness.

So your allergy statement may read: "Allergy statement: Menu items may contain or come into contact with WHEAT, EGGS, TREE NUTS, and MILK. For more information, please speak with a manager."

But the results of a total IgE test don't show what you're allergic to or how serious your allergy may be. A specific IgE test result that is high means that you may be allergic to the allergen that was tested.
