Assign a Prerequisite Click the plus sign next to the course(s) that you believe fulfills the prerequisite. You can match multiple courses to one prerequisite, if applicable. If you do not have a prerequisite course to add, click I Am Not Matching Any Courses to this Prerequisite. Click Save and Exit.

Note that your programs may require additional materials for your application to be considered complete. Check with each program for more details. Once your application enters this status, it can take up to 10 business days to move to the Verified status.

Contact the registrar at each institution you attended and request one transcript be sent to CSDCAS. Provide the registrar with the following items: CSDCAS Transcript ID Form. Ask the registrar to attach it to your official transcripts.

Your transcript should include the following information: The name of your school. Your current GPA. All terms you attended school with the course names/codes and grades as well as total credits/hours earned. High school transcripts should also include your class rank.

The easiest way to get your transcript is directly from the place that you have studied. Your institution will release official academic transcripts as well as school certificates that you will be able to enquire about in order to receive them.

An official transcript is a copy of your permanent academic record and includes: A list of the courses you took. The dates you attended the school. Your major and degree.

Sample Transcript A transcript is the final document listing the courses, grades, and final average as well as other permanent student record requirements set forth in Section 126.11(b) of Commissioner's Regulations.. Important information for future employment or continuing education is contained on the transcript.

Advanced Placement Credit Indicate if you received Advanced Placement (AP) credit on the College Board Advanced Placement Exams for any of your courses.

It includes your enrollment history, grades that you got, credits earned and attempted and average of grade-point. The classes would be arranged in chronological format by the enrollment semester or the quarter. The grade-point average may be given individually for each quarter, but total cumulative average is given.

You can submit your application before your transcripts and evaluations are received. Please note, however, that we will not review your application until all of your transcripts, payments, and evaluations are received. See Completing Your CSDCAS Application for all requirements.
