Traditionally, a monogram reads First Name Initial, Last Name Initial, Middle Name or Maiden Name Initial. With the Last Name Initial being the larger Middle Initial. For example, if you had the name Kelsie Elizabeth Vogds, her monogram would read KVE.

To sign a PDF form, you can type, draw, or insert an image of your handwritten signature or initials. Open the PDF document or form in Acrobat or Reader, and click Fill & Sign in the right pane. Professionals usually place reference initials on the bottom of a business letter, two lines after the writer's signature. Try to format the initials on the left side of the page. If there's an enclosed note or a postscript, you can place those elements underneath your reference initials.
