Proof of Ohio Domicile as of the first day of the term of enrollment: Signed copies of Rental agreement, lease, and/or HUD settlement statement of property owned. If student is not listed, a notarized statement from the person whom they are residing with is also required.

Photo Identification Driver license. US passport or passport card. US military card (front and back)

The State IDs used to cost between $10-19 to get. Now, those fees are completely waived and getting an ID is free. To obtain a State ID, people will need proof of legal name, date of birth, legal presence, social security number and Ohio residence. A full list of what is required can be found here.

Students must present their passport, I-20 or DS-2019, I-94 and proof of residency (e.g. a copy of your bank or credit card statement). The process can take 3-5 weeks.

Documents must contain the applicant's name and Social Security Number. Illinois Driver's License Record* ? Illinois ID Card Record* ? Pay stub or printed electronic deposit showing the applicant's name and SSN. Social Security Award Letter (primary recipient only)? Social Security Card (issued by SSA)

Acceptable Documents Full legal name. Date of birth. Legal presence in the US. Social Security number (SSN) Ohio street address ? TWO documents, from different sources, proving Ohio street address that are listed on the Acceptable Document List are required for proof of Ohio residency. Proof of name change (if applicable)

Hard copy documents are required; images from a cellphone are not accepted. An applicant may print the image (e.g., bank statement) and provide the printed copy when presenting documentation. Photocopies will not be accepted. All documents must contain full residence address.
