Wisconsin Notice of Termination of Limited Appearance is a form used by an attorney to withdraw from an active case in which he or she has been representing a client. This form is used to notify the court that the attorney is no longer representing the client, and that the client will need to find another attorney to continue the case. There are two types of Wisconsin Notice of Termination of Limited Appearance: voluntary withdrawal and involuntary withdrawal. Voluntary withdrawal occurs when the attorney files a Notice of Termination of Limited Appearance with the court voluntarily, usually due to a conflict of interest or a lack of time to complete the case. Involuntary withdrawal is when the court orders the attorney to withdraw from the case due to some violation of court rules or ethical standards. The Wisconsin Notice of Termination of Limited Appearance must contain the attorney’s name, address, and bar number; the client’s name and address; the name of the court; the case number; a statement that the attorney is withdrawing from the case; and the date of the filing. The Notice must also be signed by the attorney and filed with the court.
