The Washington IOWA Notice-Attachment for Additional Child is a document that must be completed whenever an additional child is added to a case involving the Indian Child Welfare Act (IOWA). This document serves as an attachment to the IOWA Notice and provides information about the additional child’s connection to the Indian Tribe, as well as any known biological and/or adoptive parents. The Washington IOWA Notice-Attachment for Additional Child includes two types of forms: Form A — Additional Child Information Form and Form B — Indian Child Information Form. Form A is used to provide information about the additional child, including their relationship to the Indian Tribe, known biological and/or adoptive parents, and other information as needed. Form B is used to provide information about the Indian child, including their connection to the Indian Tribe, known biological and/or adoptive parents, and other information as needed. The Washington IOWA Notice-Attachment for Additional Child must be signed by all parties involved and served on the Indian Tribe.
