Texas Individual Short-Term Recovery Care Checklist is a form developed by the Texas Department of State Health Services (DSS) to help individuals with mental health needs to plan for short-term crisis recovery care. It is designed to ensure that individuals receive safe, effective, and appropriate care during a crisis. The Checklist includes questions about the individual's mental health and substance use history, current mental health symptoms, current support system, and possible treatment and support options. It also includes a list of potential recovery resources, including support groups, crisis hotlines, and community mental health centers. The Checklist is made up of two distinct types: the Adult Checklist and the Youth Checklist. The Adult Checklist is designed for individuals 18 years and older, while the Youth Checklist is designed for individuals 17 years and younger.
