Tennessee Sole Proprietor/Partner Withdrawal of Election Form I-5 is a form used to make a withdrawal from the election to be taxed as a Tennessee sole proprietor or partner. This form is filed with the Tennessee Department of Revenue and must be signed by the taxpayer, and any partners if the taxpayer is a partnership. The form allows the taxpayer to revoke the election to be taxed as a Tennessee sole proprietor or partner and to be taxed as a regular corporation instead. There are two types of Tennessee Sole Proprietor/Partner Withdrawal of Election Form I-5: Form I-5A for revoking the election to be taxed as a Tennessee sole proprietor and Form I-5B for revoking the election to be taxed as a Tennessee partner.
