The Rhode Island Civil Cover Sheet is a form used in the state of Rhode Island for initiating civil cases in the state's Superior Court. The cover sheet is used to provide the court with basic information about the case, including the names and addresses of the parties, the type of case being filed, the amount of damages sought, and the name of the judge to whom the case is assigned. There are three types of Rhode Island Civil Cover Sheet: the Civil Cover Sheet for General Civil Actions, the Civil Cover Sheet for Domestic Relations Actions, and the Civil Cover Sheet for Small Claims Actions. The Civil Cover Sheet for General Civil Actions is used for all civil cases that do not involve domestic relations or small claims. The Civil Cover Sheet for Domestic Relations Actions is used for cases related to divorce, child custody, alimony, and other family law matters. The Civil Cover Sheet for Small Claims Actions is used for cases involving monetary damages of $2,500 or less.
