Pennsylvania Notice (Voluntary Relinquishment) is a document used in the state of Pennsylvania to terminate parental rights. This document allows a parent to voluntarily give up their parental rights and responsibilities without the need for a court order. It is used when a parent wishes to permanently end their relationship with their child without going through the court system. There are two types of Pennsylvania Notice (Voluntary Relinquishment): one for adults and one for minors. An adult must sign the Adult Voluntary Relinquishment Notice if they are 18 years of age or older. The Minor Voluntary Relinquishment Notice must be signed by a minor's parent or guardian if the minor is under the age of 18. Both forms must be signed by both the parent relinquishing their parental rights and the person accepting the relinquishment of those rights. Once the form is signed and notarized, the parent's rights are legally terminated.
