This form is used when the lawyer is withdrawing from a case and the client is representing themselves, known as pro SE. There are two types of the form. The first is the Attorney Withdrawal, which is used by the lawyer when they are no longer representing the client in a matter. The second is the Pro Se Entry, which is used when the client is representing themselves in a case. This form contains basic information about the case, including the parties involved, the court and the date of the filing. The lawyer and client must both sign the form to make it official.
