The Oregon Adversary Proceeding Cover Sheet is a document filed with the Oregon Bankruptcy Court that provides basic information about a bankruptcy case. It is used to initiate a bankruptcy action, or adversary proceeding, and must be completed and filed with the Court prior to any adversary proceeding being commenced. The cover sheet includes the names of the parties involved, the bankruptcy case number, the chapter of the Bankruptcy Code under which the case is being filed, the title of the action, and a brief summary of the relief sought. There are two types of Oregon Adversary Proceeding Cover Sheets: the standard cover sheet and the motion cover sheet. The standard cover sheet is used for adversary proceedings that arise during the course of a bankruptcy case, such as a motion to lift the automatic stay, a motion to dismiss the case, or a motion for relief from the stay. The motion cover sheet is used for adversary proceedings that are initiated separately from an existing bankruptcy case, such as an adversary complaint.
