The Oklahoma Certificate of Original Record (COR) is a document issued by the Oklahoma State Department of Health, Vital Records Service, which contains a copy of an individual's birth certificate or marriage certificate. It is an official document that serves as a primary source of identification and is often used to prove identity for various legal purposes. There are two types of Oklahoma Certificate of Original Records: the Long Form COR, which contains information such as the individual's full name, date of birth, place of birth, parents' names, and a signature of the attending physician; and the Short Form COR, which does not have any of the detailed information included in the Long Form COR. Both types of COR's can be used to obtain a driver's license, passport, or social security card.
