The Ohio Notification Form is a required document that must be completed and submitted by all vendors and contractors who have been awarded a contract with the State of Ohio. The purpose of this document is to notify the Ohio Department of Taxation of any changes in the contractor’s business name, address, or contact information. The Ohio Notification Form must be submitted prior to the performance of any contract services. There are two types of Ohio Notification Forms: the Ohio Business Notification Form and the Ohio Contractor Notification Form. The Ohio Business Notification Form is for vendors and contractors who are conducting business with the State of Ohio as an individual, partnership, or corporation. The Ohio Contractor Notification Form is for vendors and contractors who are performing construction or maintenance services for the State of Ohio.
