The Ohio Vital Statistics Certificate of Adoption is an official document issued by the Ohio Department of Health (ODD), that certifies the adoption of a child in the state of Ohio. The certificate provides legal evidence of the adoption in the state of Ohio and establishes the adoptive parents as the legal parents of the child. There are two types of Ohio Vital Statistics Certificates of Adoption: Long Form Certificates and Short Form Certificates. The Long Form Certificate includes the child's original birth name, date of birth, and sex, as well as the adoptive parents' names and the date of adoption. The Short Form Certificate provides the same information as the Long Form Certificate but does not include the child's original birth name. Both certificates are issued with a certified seal, and are valid as evidence of adoption in the state of Ohio.
