The New York Affidavit of Service by Mail of JOB (Form UD-15) is a document used to prove that a court document was mailed by first-class mail to the proper party. The form is signed by the party mailing the court document, and it includes the name and address of the party to whom the document was mailed, as well as the date and method of service. There are three types of New York Affidavit of Service by Mail of JOB (Form UD-15): Process Server Affidavit of Service by Mail, Personal Service Affidavit of Service by Mail, and Publication Affidavit of Service by Mail. Each of these affidavits must be properly completed and notarized in order to be accepted by the court.
