Financial Disclosures: This section requires parents to provide information on all assets, debts, income, and expenses. This includes bank accounts, investments, real estate, and other financial documents. Other Disclosures: This section requires parents to provide any other information or documents that may be relevant to the parenting case. This includes any information about the fitness of either parent to act as a parent, any previous child custody orders, any criminal convictions, any domestic violence convictions, any substance abuse or mental health treatment, any evidence of abuse or neglect, any paternity or parentage issues, and any protective orders.
