Minnesota Affidavit of Service — Reinstatement of License is a document used to prove that a driver has submitted the appropriate paperwork and fees to reinstate their driver's license after it has been suspended, revoked, or canceled. It generally includes the driver's name, address, license number, and the date of service. There are two types of Minnesota Affidavit of Service — Reinstatement of License: 1) an affidavit for the reinstatement of a suspended, revoked, or canceled driver's license and 2) an affidavit for the reinstatement of a driver's license after a conviction for Driving Under the Influence (DUI) or Driving While Impaired (DWI). The affidavit must be signed and notarized before it is submitted to the Minnesota Department of Public Safety.
