There are two types of Indiana 16-1. Appealed's Notice of Appearance: the pre-printed version and the free-form version. The pre-printed version contains a set of standardized questions and is best used for simple civil cases. The free-form version allows the attorney to enter customized information and is best used for more complex civil cases.
