The Illinois Itemization of Services Rendered To Chapter 13 Debtor(s) is a document that is used by a Chapter 13 debtor's attorney to itemize and document the services provided to the debtor. It includes a detailed list of all services performed by the attorney, including research, preparation, review, and filing of documents, as well as any other services rendered during the Chapter 13 process. The document must be signed by the debtor and the attorney. There are two types of Illinois Itemization of Services Rendered To Chapter 13 Debtor(s): the initial itemization of services and the amended itemization of services. The initial itemization should be submitted to the court at the time of filing of the Chapter 13 petition. The amended itemization of services should be filed when the debtor’s attorney has provided additional services or has modified the services listed in the initial itemization.
