California Proof of Firearms Turned In, Sold, or Stored — Civil Harassment Prevention is a form of identification provided by the California Department of Justice (DOJ) to individuals who possess firearms and wish to turn them in, sell them, or store them for safety. This form is used to ensure the firearms are not used in any criminal activity and to verify the identity of the person turning in, selling, or storing the firearms. There are two main types of California Proof of Firearms Turned In, Sold, or Stored — Civil Harassment Prevention: the Voluntary Surrender Form and the Civil Harassment Prevention Form. The Voluntary Surrender Form requires the individual to surrender the firearm to the sheriff or police department, as well as provide proof of identity and a description of the firearm. The Civil Harassment Prevention Form requires the individual to provide the same information as the Voluntary Surrender Form, but also requires them to state why the firearm is being surrendered or stored and sign an affidavit of civil harassment prevention.
