An Arizona Statement of Bankruptcy or Receivership Other Corporation Attachment* is a document that a corporation must file with the Arizona Secretary of State when it is filing bankruptcy or receivership in Arizona. This document must be filed along with the Bankruptcy or Receivership Petition and includes the name and address of the filing corporation, the name and address of the court in which the petition is being filed, the case number, and the name of the bankruptcy or receivership trustee appointed by the court. There are two types of Arizona Statement of Bankruptcy or Receivership Other Corporation Attachment*: the “Attachment A” and the “Attachment B”. The Attachmenrequirementsires the filing corporation to list the names and addresses of all officers, directors, shareholders, and members of the corporation. The Attachment B requires the filing corporation to list the names and addresses of all creditors and their respective claims.
