Arizona Request for Excuse (ARE) is a form used by employers in Arizona to excuse an employee from jury duty. The form provides employers with an opportunity to explain why the employee should not be required to serve jury duty. There are two types of ARE's: Request for Excuse due to Financial Hardship and Request for Excuse due to Other Reasons. The Financial Hardship Request requires employers to provide proof of financial hardship such as pay stubs, tax returns, or other proof of income. The Other Reasons Request requires employers to provide an explanation of why the employee should be excused from jury duty. The form must be signed and dated by the employee, the employer, and the court.
