Alaska Instructions and Request for Records — All Other Locations is a set of instructions issued by the Alaska Department of Health and Social Services (DSS) regarding the process to request records from any location other than Alaska. This includes records held by entities such as private organizations, Native corporations, or other governmental entities. The instructions outline the procedures for submitting a request for records, including the documents that must be included, the fees associated with the request, and the methods of payment accepted. There are two types of Alaska Instructions and Request for Records — All Other Locations: 1) Request for Medical Records; and 2) Request for Non-Medical Records. The request for Medical Records is used when requesting medical records from entities in other locations while the request for Non-Medical Records is used when requesting records other than medical records.
