The 1004D Virtual Completion Certificate offers a convenient and swift way to confirm the fulfillment of appraisal report conditions for construction or repairs/alterations. A seamless connection between the appraiser and the property contact is established through a video streaming service.
Fannie Mae Form 1004D/Freddie Mac Form 442, “Appraisal Update and/or Completion Report” is to be used to extend the validity period. When an appraisal is obtained, the property must be appraised within the 12 months that precede the date of the note and mortgage.
Completion Report means the final report prepared by the Consultant at the completion of the Services (for detailed requirements refer to Appendix 1, Scope of Services).
The 1004D Appraisal Form, or Completion Report, is a document used by lenders when a home appraisal suggests repairs or inspections are needed. It confirms that these issues have been fixed. This report helps reassure both lenders and first-time home buyers that necessary fixes have been done properly.
Note: Interior photographs on proposed or under construction properties may be taken at the time of the completion inspection and included with Form 1004D. At a minimum, when completing the Appraisal Update portion of the report, a photograph of the front of the subject property must be included.
Appraisal Update and/or Completion Report Sequence Loading In the event an Appraisal Update and/or Completion Report (Fannie Mae Form 1004D / Freddie Mac Form 442) is required for an appraisal, it must be loaded in the next sequential Appraisal field (Appraisal 2 or 3).
11) 1004D: Final Inspection and/or Recertification of Value The report is considered a new appraisal assignment as a follow up to an originally performed Conventional or FHA appraisal. For a Final Inspection, the appraiser will indicate and verify that the conditions of the original report have been met or cured.
