Bidders must complete and submit all of the following documents with the Bid Form and Proposal: a. The quantities in the proposal are approximate. Each bid must be submitted in a sealed envelope. Bid insert pages must be returned with the bid proposal submitted to the Department, unless your firm is submitting a bid using a computer print out. HUB form(s)1, 2, and 6 must be filled out completely, signed, and returned with the bid.
