The Jamaica C5 Form is the online version of the Immigration and Customs Declaration required for all air passengers entering Jamaica. It allows air passengers to submit their travel and personal details online in advance.

All visitors are required to be in possession of a national passport or other acceptable travel document establishing nationality and identity, and bearing a photograph. The passport or other travel document must be valid for the period of intended stay in the island.
All individuals entering Jamaica by air, regardless of nationality or residency status, are required to complete the C5 Form. This includes: Jamaican citizens returning to the island. Non-residents traveling for leisure, business, or other purposes.
