You will need to complete 75 hours of home healthcare training. This will include 16 hours of clinical or practical training where you will be supervised. Twelve hours of continuing education training is also required every 12 months. After your home healthcare training is complete, you must pass a competency exam.
In Ohio, you can register as a transportation provider if you provide the state's medicaid office with documentation, such as your driver's license; a vehicle maintenance report; and a written backup plan in the event that your vehicle breaks down. You also have to pass a drug screening and a competency test.
The long answer: Ohio requires that all parties who do not contract directly with the real property owner (except laborers) must file a Notice of Furnishing in order to protect their lien rights. This includes subcontractors, material suppliers, and anyone who isn't an original contractor or a principal contractor.

The property owner must sign the Notice of Commencement. If there are multiple property owners, the NOC may only require one signature, but it is recommended that all legally registered property owners add their signatures, as well. In some states, you will also need to have the form notarized.
