If you are seeking to be permanently excused, mail the summons, the completed update card, a full note of explanation and any supporting documentation, such as a doctor's note, certificate of prior jury service or proof of a new address (yellow postal forwarding sticker, copy of a new drivers license or utility bill).
An individual can request an exemption or postponement from their jury duty in New York, provided they have a legitimate reason for being unable to serve. As such, they should contact the court promptly to explain their situation, submit necessary documents, and follow through the required procedures.
If you cannot serve even if granted a postponement, you may contact your local commissioner of jurors office and ask to be excused from service. The commissioner may ask you to provide documentary proof of the reasons why you need to be excused.
You have a medical condition or disability A diagnosis of your mental or physical condition. A prognosis of how long the condition is expected to exist. A conclusion stating that you are incapable of serving as a juror currently or in the future.

For example, write something like “My name is Jane Doe. On (date), I received a jury summons for (court date) at (court address) and was assigned the juror number (insert number here). I am writing to request to be excused from jury service because being absent from work would pose an extreme financial hardship.”
Sole parents, main breadwinners, leader on a big project at work, physical or mental disabilities that may impede your attendance... these things will almost always get you out of Jury Duty, though you should try to bring documentation of some kind to prove it.
If the date your are summoned is inconvenient, the law allows you one postponement to an available jury term date of your choice within six months. Requests to be postponed must be made as soon as possible to the Jury Commissioner's office at 607-216-6622.
Absent extraordinary circumstances, the commissioner shall not grant a prospective juror more than three postponements of jury service, nor shall the aggregate period of postponements granted to a prospective juror exceed 18 months.
