Beginning in 2024, the IRS allows electronic filing of Form 941-X as part of Modernized e-File (MeF). However, only corrections for the 2024 tax year could be e-filed.
Fill out the “Return You're Correcting” section in the upper right corner of Form 941-X. Enter the calendar year of the quarter you're correcting, and select the date you discovered errors in your Form 941. Under Part 1, select whether you're filing an updated job tax return (box #1) or a claim (box #2).
When you discover an error on a previously filed Form 941, you must: Correct that error using Form 941-X; File a separate Form 941-X for each Form 941 that you're correcting; and. Generally, file Form 941-X separately. Don't file Form 941-X with Form 941.
If you made an error that requires a Form IL-941-X and you made the same reporting error on your return and your employee's Form W-2 or payee's Form 1099 or W-2G, you must also provide your employee or payee with a Form W-2c or corrected Form 1099 or W-2G, and attach copies to your Form IL-941-X.
If you discover an error on a previously filed Form 941 or need to amend a Form 941, you would correct using Form 941-X, Adjusted Employer's Quarterly Federal Tax Return or Claim for Refund. This is filed separately from Form 941.

The Sequence Number is usually ”-000”, but can sometimes be “-001.” Most new employers in Illinois will have the “-000” Sequence Number. If you do not know your Sequence Number, call the IL DOR at (800) 732-8866.
Sometimes you may make a mistake on your initial Form 941 that you need to correct. You'll then use Form 941-X to amend the original filing. This amendment form can also be used to claim credits retroactively if you are eligible but failed to claim it for the applicable quarter.
