Corrections made with white-out, crossing out the incorrect information, writing the correction on top of the error, etc., are not acceptable.
To correct a data or printing error, submit the following by mail: Form DS-5504 (find mailing instructions on the form) Your current passport. One color photo. Evidence of the error (e.g. your U.S. birth certificate showing the correct spelling of your last name). You will not have to submit any fees.
Fill in personal details, reasons for change, and include supporting evidence. The affidavit must be notarized to lend it legal legitimacy. Remember to include details of the changes, a statement of truth, and the affidavit's execution date and location.
Entry of applicants into the Passport Office is not prohibited to make any correction in their documents or to get any updates. If any correction is needed after submission of the application, applicant can visit enquiry counter in the RPO concerned either by scheduling an appointment online or walk-in.

Please take your time and print carefully. Passport Services is no longer accepting applications with items crossed out and corrections written above or beside it. A new application will have to be completed. “White Out” and cross outs are not accepted.
