This cover sheet is used to inform the court or agency of the case number, names of parties involved, type of case, amount in dispute, and other important information. The cover sheet is also used to provide contact information for the plaintiff, defendant, and counsel. There are two types of New Jersey False Claims Act Cover Sheet: the Complaint Cover Sheet and the Answer Cover Sheet. The Complaint Cover Sheet is used by the plaintiff when filing a complaint against the defendant with the court or agency. The Answer Cover Sheet is used by the defendant when responding to the plaintiff’s complaint. Both forms are available on the New Jersey Judiciary website.
