Maryland Uniform Criminal Citation — Sample is a form issued by the Maryland State Police for the purpose of charging an individual with a criminal offense. It is a standardized document that includes the offense, the date and time of the offense, the name and address of the accused, the arresting officer's information, and the arresting agency's information. The citation also includes the signature of the arresting officer and a notice that the accused must appear in court. There are two types of Maryland Uniform Criminal Citation — Sample: one is dated and the other is undated. The dated version includes the date of the offense and the undated version does not.
