Students must then have a physician complete the physician's form certifying that the student is physically able to perform the work. If a student is an athlete and has a current physical on file with the district, this can be used in lieu of having a physician complete a form.

Proof of Age document is required. This could include a driver's license or birth certificate. The student must return the completed forms and sign the original work permit and take it to their employer. Also, an extra signed copy along with the application will remain in their folder in Student Services.
