DescriptionThis Advance Health Care Directive was enacted in the Hawaii Revised Statutes 372E-16. It is a combination of forms, all dealing with health care issues, including power of attorney for healthcare, healthcare instructions, anatomical gifts, and designation of primary physician. It allows you to provide instructions regarding your health care, name someone else to make health care decisions for you and express your wishes regarding designation of a health care provider. You may modify the statutory form to suit your individual needs or use a different form that contains the substance of the statutory form. Your advance health care directive must be in writing, dated, signed, and witnessed by at least two individuals (each of whom witnessed either the signing of the instrument or the acknowledgment of the signature of the instrument) or acknowledged before a notary public at any place within Hawaii. | ||||||||||||||||||||||
