A set of articles of incorporation is a document that must be filed with a state in order to incorporate. Among the things it typically must include are the name and address of the corporation, its general purpose and the number and type of shares of stock to be issued. The first document in this example is a set of articles of incorporation for a business corporation, meeting the requirements of La. R.S. 12:24. The second document is the initial report in compliance with La. R.S. 12:101, filed with the corporate articles of incorporation. The third document is an affidavit of acceptance of appointment executed by the corporation's appointed agent for service of process and attached to the initial report of the corporation as required by La. R.S. 12:104C.(2). (Where the agent for service of process is itself a corporation, the affidavit must be executed by a duly authorized officer of the corporation.)
