Wisconsin Notice to Victims (Juvenile Court Intake) is a form that is used by the Wisconsin Department of Children and Families in order to inform victims of juvenile court proceedings. This notice is issued to victims of juvenile delinquency, i.e. minors charged with criminal offenses, and provides victims with information on their rights and available resources. The Notice to Victims includes the name of the child, court date, contact information for the District Attorney’s Office, and the right to be informed of all court proceedings. Victims may also be provided with information on counseling and other resources available to them. There are two types of Notice to Victims (Juvenile Court Intake) forms: the Initial Notice to Victims and the Continuation Notice to Victims. The Initial Notice is sent to victims when the child is first charged with an offense; the Continuation Notice is sent when the case is continued or when there is a change in the court proceedings.
