The Rhode Island Statement of Change of Specified Office and Or Registered Agent is a document that must be filed with the Rhode Island Secretary of State when a corporation or business entity wants to change their registered office address or their registered agent. This document is also known as the Statement of Change of Registered Office and/or Registered Agent and is available on the Secretary of State's website. There are two types of Rhode Island Statement of Change of Specified Office and Or Registered Agent: one for changing the registered office address and one for changing the registered agent. Both forms require the entity's name, the new address of the registered office, the name and address of the registered agent, and the date of the change. The document must be signed and dated by an authorized representative of the entity before being filed with the Secretary of State.
