Order to Notify Illinois State Police of an Adjudication of a Person as Mentally Disabled is a legal process by which a court issues an order to the Illinois State Police to inform them that the court has adjudicated a person as mentally disabled, as defined by the Mental Health and Developmental Disabilities Code. This order is generally used when a person is found to be unable to manage their own affairs due to a mental disability or illness. There are two types of Order to Notify Illinois State Police of an adjudication of a Person as Mentally Disabled: 1. Order to Notify (OTN) — This order is issued by the court when a person is found to be incapable of managing their own affairs due to a mental disability or illness. The OTN is sent to the Illinois State Police to be entered into the Firearm Owner Identification (FOOD) card database. 2. Order for Release of Firearm (OF) — This order is issued when a person is found to be capable of managing their own affairs due to a mental disability or illness but is still prohibited from owning firearms. The OF is sent to the Illinois State Police to be entered into the FOOD card database.
