The Arizona Disclosure of Compensation of Bankruptcy Preparer is a document that outlines the fees and other compensation a bankruptcy preparer may receive for providing bankruptcy services to a debtor. This disclosure must be provided to debtors prior to the commencement of services and must be signed and dated by both the debtor and the bankruptcy preparer. The Arizona disclosure of compensation must include the type of services provided, the fee amount, any additional charges, and the method of payment. There are three types of Arizona Disclosure of Compensation of Bankruptcy Preparer: Full Disclosure, Limited Disclosure, and No Disclosure. The Full Disclosure must include an itemized list of services, fees, and other compensation. The Limited Disclosure must include an itemized list of services, fees, and other compensation, along with a description of the services and a statement of the total fees. The No Disclosure option does not require any additional information outside the fee amount.
