Colorado Affidavit of Labor or Materials Furnished - Individual or Corporate

This document is an affidavit executed by a party who has furnished labor and materials to the contractor and the contractor's bond surety that materials and labor have been supplied. To be effective it must be served personally, or through certified or registered mail. The contractor has forty-five days from receipt to controvert said affidavit or his surety shall pay the claim in full.
