- For section 'V', provide the amount of general excise tax, if applicable, and indicate if you paid under protest by placing an 'X' in the designated box.
- In section 'VI', state the basis of your appeal clearly and concisely. For property tax appeals, refer to Section 232-3, HRS, and attach a separate sheet if more space is needed.
- Complete the date and location at the bottom of the form, ensuring that both the appellant's or counsel’s signature fields are completed.
- Lastly, indicate the amount of court costs paid, noting that there is a $100.00 non-refundable filing fee for regular claims in any one case. Review your form for completeness and accuracy, then save any changes, download, print, or share the form as required.
