- Items 17 through 21 apply to machine ticket claims. Enter the date from your ticket in Item 17, the prize amount in Item 18, select the game in Item 19, and input the serial number from the front of your ticket for Item 20. For Item 21, use the validation slip number if available.
- For Item 22, reference separate instructions if you have a questionable claim.
- For Item 23, provide your driver’s license number and indicate if this is your claim or someone else's. Then, sign the Claim Form in Item 24 once it has been printed.
- After printing and signing the Claim Form, ensure to sign the back of your ticket(s) and staple them to the Claim Form in the indicated areas. Retain copies of all documents for your records.
- Mail the completed Claim Form along with your original ticket(s) and any validation slip to the New Jersey Lottery at the specified address.
