How to use or fill out the CG6215.pdf. How To Calculate A Significant Threshold Shift - Uscg online

This guide provides clear instructions on how to complete the CG6215.pdf, a form used to calculate a significant threshold shift in hearing. By following these steps, users can ensure accurate and efficient submission of the required information.

Follow the steps to accurately complete the form.

  1. Click ‘Get Form’ button to obtain the form and open it in the editor.
  2. Begin by entering the date of the current audiogram at the top of the section labeled 'Current Audiogram'. Fill in the values for the left and right ear at 2000 Hz, 3000 Hz, and 4000 Hz, as indicated.
  3. Proceed to the 'Baseline Audiogram' section and enter the date. Again, fill in the values for both the left and right ear at the specified frequencies.
  4. After completing both audiogram sections, compute the summed values for each ear by adding the values you entered for 2000 Hz, 3000 Hz, and 4000 Hz.
  5. Subtract the baseline audio sum from the current audio sum for each ear separately and document these values.
  6. For step 2, take the left ear difference calculated in the previous step and divide this value by 3. Repeat this for the right ear.
  7. Review the final results to determine if there has been a significant threshold shift, defined as either an average change of > ± 10 dB or a change of > ± 15 dB at certain frequencies.
  8. Once all fields are filled out and checked for accuracy, users can save changes, download, print, or share the completed form as necessary.

Complete your form online for efficient processing and accurate record-keeping.

When it comes to hearing loss, a Standard Threshold Shift (STS) is a 10 dB(HL) deterioration of the average of thresholds at 2000, 3000, and 4000 Hz when comparing the baseline for each ear to the most recent test.

An STS is defined at 29 CFR 1910.95(g)(10) in the occupational noise exposure standard as "... a change in hearing threshold relative to the baseline audiogram of an average of 10 dB or more at 2000, 3000, and 4000 Hz in either ear." This definition applies regardless of the hearing threshold values in the baseline ...

A Standard Threshold Shift, or STS, is defined by OSHA as a change in hearing threshold, relative to the baseline audiogram for that employee, of an average of 10 decibels (dB) or more at 2000, 3000, and 4000 hertz (Hz) in one or both ears.

1. Has the employee suffered a STS (Standard Threshold Shift)? The OSHA noise standard defines a STS as an average 10dB or more loss in one or both ears relative to the most current baseline audiogram averaged at 2000, 3000 and 4000 Hz. No → Stop. This is not an OSHA recordable hearing loss.

THRESHOLD SHIFT (TS) On exposure to NOISE, the ear's sensitivity level will decrease as a measure of protection. This process is referred to as a shift in the THRESHOLD OF HEARING, meaning that only sounds louder than a certain level will be heard. The shift may be temporary, chronic or permanent.

Question: How is an STS calculated? Answer: An STS is calculated by comparing the annual audiogram to the baseline audiogram at 2,000, 3,000 and 4,000 Hz. If an average difference of 10 dB or more has occurred at these frequencies, an STS has occurred. The average change is equal to -5 + 5 + 15 = 15 = 5 dB.
